The most reliable contraceptive methods are long-acting: the intrauterine device and the subdermal implant give fewer than 1 pregnancy per 100 women per year, and almost regardless of the person, because there is nothing to forget. Pills, used perfectly, are also very reliable, but in real life they fail more often. Here are the methods by honest, "typical" effectiveness and how to choose by your priorities. This is a guide, not a prescription: the method is chosen with a doctor based on your health.

How to read effectiveness

The key number for any method is not "what percentage it protects" but the pregnancy rate over a year of real use. And here it is important to distinguish two figures.

  • Perfect use — if everything is done flawlessly.
  • Typical use — as it works out in life: missed pills, torn condoms, forgotten days.

For methods that depend on the person every day or every time, the gap is large. For "fit and forget" methods it is almost nil — which is exactly why they lead on reliability.

Hormonal methods

These include combined methods (estrogen + progestin): the pill, patch and vaginal ring — and progestin-only ones: the mini-pill, injection, implant and hormonal IUD. They suppress ovulation and/or change mucus and the endometrium.

Beyond contraception they often carry bonuses: less painful and lighter periods, help with acne, PMS, endometriosis and PCOS. For many this is a second reason to choose them.

Non-hormonal methods

  • Copper IUD — as reliable as the hormonal one (under 1%) but hormone-free; it may make periods heavier.
  • Condom — the only method that also protects against infections; moderately reliable for pregnancy.
  • Fertility-awareness methods — no device at all, but they require discipline and give a wide range of effectiveness.

An important caveat: ovulation calculators and apps show the fertile window only approximately and are not intended for contraception — their margin of error is too large to rely on for preventing pregnancy.

How to choose

There is no universally "best" method — there is the best one for your priorities:

  • Maximum reliability and "fit and forget" → implant or IUD.
  • Protection against infections → condom (can be paired with another method).
  • A bonus for skin and cycle → combined methods (if there are no contraindications).
  • Hormone-free → copper IUD or barrier methods.

Emergency contraception

If sex was unprotected, there are two options: emergency contraception pills (more effective the sooner they are taken — within 72–120 hours depending on the product) and a copper IUD fitted within 5 days (the most reliable option). This is a one-off measure for a lapse, not an everyday method.
