Triglycerides: Normal Range and How to Lower Them

What triglycerides measure, which values count as normal or high, why levels above 500 mg/dL are urgent, and which diet and lifestyle changes actually move the number.

Triglycerides are the most volatile line on a lipid panel and the most responsive to how you live. Where LDL is largely genetic and shifts only modestly with diet, triglycerides can halve within a couple of months of dropping alcohol and sugary drinks. That makes them an excellent indicator of metabolic state — and it is also why they are so often overlooked by people who look only at "total cholesterol".

What they are

Triglycerides are the body's main storage form of fat. They arrive with food and are synthesised in the liver from surplus carbohydrate and alcohol, circulate inside very-low-density lipoproteins (VLDL) and chylomicrons, and are then either burned by muscle or deposited in adipose tissue.

On a blood test, triglycerides reflect the balance between intake, synthesis and clearance. A raised level almost always signals either more energy coming in than going out, or impaired insulin sensitivity — insulin normally suppresses fat breakdown and activates lipoprotein lipase, and that mechanism falters when resistance develops.

Why they matter

Triglycerides were long treated as secondary to LDL. The picture is now more nuanced. Triglycerides themselves do not deposit in artery walls, but the particles that carry them — remnant lipoproteins — are small enough to enter the vessel intima and drive atherosclerosis. High triglycerides also typically travel with low HDL and small dense LDL particles: the atherogenic dyslipidaemia characteristic of metabolic syndrome and type 2 diabetes.

A separate and far more acute issue is pancreatitis. Above 10 mmol/L, chylomicrons overload the plasma to the point of triggering acute inflammation of the pancreas. That needs immediate medical care, not a month of dietary adjustment.

Alcohol deserves special mention: in people with already-high triglycerides, even a few drinks a week keep the number elevated, and a single heavy session can cause a sharp spike. At very high levels, complete abstinence is the first and most effective step.

The encouraging part is that triglycerides respond faster than any other lipid measure — the effect is visible within 4–8 weeks.

One nuance about carbohydrates: cutting them is not uniformly effective. In people with insulin resistance, replacing refined carbohydrates with whole grains and legumes produces a clear improvement. But an abrupt shift to a very low-carbohydrate, high-saturated-fat diet may lower triglycerides while raising LDL — winning on one marker and losing on another. Changing carbohydrate quality is smarter than eliminating carbohydrates.

Medication

For people at high cardiovascular risk the mainstay is statins, which lower both LDL and triglycerides by 10–30 %. Above 5.6 mmol/L fibrates may be added — their main job is preventing pancreatitis rather than reducing atherosclerotic risk. Prescription icosapent ethyl (highly purified EPA) reduced cardiovascular events in REDUCE-IT among statin-treated patients with high triglycerides, though the contribution of the mineral-oil placebo remains debated.

Triglycerides are a fast, honest readout of metabolic state. Optimal is under 1.7 mmol/L; higher values mean either more energy in than out, or impaired insulin sensitivity. The main levers are alcohol, sugar and refined carbohydrates, body weight and aerobic activity — together capable of a 20–50 % reduction within a couple of months. At very high levels the concern shifts from atherosclerosis to pancreatitis, and that requires a doctor.

FAQ

What is a normal triglyceride level?+

Optimal is under 1.7 mmol/L (150 mg/dL). 1.7–2.3 mmol/L (150–199 mg/dL) is borderline high, 2.3–5.6 (200–499) is high, and above 5.6 mmol/L (500 mg/dL) is very high, with real risk of acute pancreatitis.

Do I need to fast before the test?+

Current European guidance accepts non-fasting lipid panels for routine risk assessment. But if non-fasting triglycerides exceed 4.5 mmol/L (about 400 mg/dL), the test is repeated after a 12-hour fast, since triglycerides shift after meals more than any other lipid measure.

Why do high triglycerides matter?+

Moderately raised levels are associated with higher cardiovascular risk, as a marker of triglyceride-rich lipoproteins and the insulin resistance that usually accompanies them. Above 10 mmol/L (885 mg/dL) the risk of acute pancreatitis rises sharply — that is an emergency, not a diet project.

What lowers triglycerides fastest?+

Cutting out alcohol, restricting sugar and refined carbohydrates, losing 5–10 % of body weight, and regular aerobic exercise. Together these can drop the number 20–50 % in 4–8 weeks — a bigger swing than any lifestyle change produces for LDL.

Do omega-3s help?+

Yes, but only at high doses. Standard 1 g fish oil capsules do little; meaningful reduction requires 2–4 g of EPA/DHA per day, which is a prescription-level decision. Prescription icosapent ethyl also reduced cardiovascular events in the REDUCE-IT trial.
