Do Multivitamins Do Anything for Healthy People?

The USPSTF and large trials found no benefit of multivitamins for healthy adults. Who genuinely needs supplements, and where upper limits actually bite.

If you are a healthy adult eating a reasonably varied diet, a daily multivitamin will almost certainly do nothing measurable for you: it does not lower overall mortality, does not prevent heart attacks or strokes, and does not reduce cancer incidence. That is the conclusion of the US Preventive Services Task Force in 2022 and of the systematic review underpinning it. But the honest version of this story has a second half, because several groups of people genuinely do need specific supplements, and for one of them — anyone who might become pregnant — the recommendation is not optional.

What the large trials actually found

In June 2022 the USPSTF updated its guidance on vitamin and mineral supplements for the primary prevention of cardiovascular disease and cancer. The evidence base was a systematic review by O'Connor and colleagues, published in the same issue of JAMA: 84 studies, 78 of them randomized controlled trials, together covering hundreds of thousands of participants.

For multivitamins the picture was flat. A couple of large trials hinted at a small reduction in cancer incidence, but the signal was inconsistent across studies and was not accompanied by any reduction in cancer mortality, cardiovascular events or all-cause death. The Task Force's formal wording is that the evidence is insufficient to weigh benefits against harms — an "I" statement, not a green light.

Two nutrients got a firmer verdict. The USPSTF recommends against beta-carotene and vitamin E for preventing cardiovascular disease or cancer. Vitamin E showed no benefit; beta-carotene showed active harm. That is a grade D recommendation, which is unusual — a public body saying "do not do this" rather than "we do not know".

The scope matters as much as the finding. These statements apply to community-dwelling, non-pregnant adults without a known deficiency. They do not override treatment of diagnosed deficiencies, do not apply to people with malabsorption, and sit alongside a completely separate USPSTF recommendation on folic acid for anyone planning a pregnancy.

Why the null result is not surprising: multivitamins were invented as insurance against deficiency diseases — scurvy, beriberi, pellagra. In countries with fortified flour, iodized salt and year-round produce, those diseases mostly vanished while the pill stayed. Curing a deficiency and improving the health of someone who has none are simply different problems, and the second one turned out to be much harder.

Who genuinely needs supplements

This is the practically useful part of the topic, and it usually gets lost in arguments about whether vitamins "work".

The first row deserves emphasis because it outweighs everything else on the list. Taking 400–800 mcg of folic acid before conception and through early pregnancy reduces the risk of neural tube defects such as anencephaly and spina bifida. It carries a grade A recommendation — not "consider it", but "everyone who could become pregnant should take it". The timing is the whole point: by the time a test reads positive, the critical window has largely closed.

Why "just in case" is a poor strategy

"It can't hurt" sounds harmless. It has three specific problems.

Vitamins have ceilings. Most nutrients have a tolerable upper intake level, and a single multivitamin rarely comes close to it. Trouble starts when someone takes a multivitamin plus a separate vitamin D, plus zinc "for immunity", plus fortified cereal and protein bars. Nobody adds these up.

Some effects are not merely null but negative. This is not a hypothetical; it comes from large randomized trials that were designed to demonstrate benefit and found the opposite.

A pill substitutes for the things that matter. Swallowing something feels like taking action and relieves the anxiety, while smoking, sleep, movement and diet stay untouched. That displacement effect is hard to measure but real.

Other places it is easy to overshoot

  • Iron. The body has no active route for excreting iron. Supplementing without a confirmed deficiency drives overload, which is particularly dangerous in hereditary haemochromatosis — roughly 1 in 200 people of European descent. Men and postmenopausal women should not take iron without a ferritin result.
  • Selenium. A narrow window: 55 mcg a day is the requirement, 400 mcg the upper limit. Chronic excess causes selenosis — brittle hair and nails, a garlic odour on the breath, peripheral neuropathy.
  • Vitamin D. Safe at sensible doses, but repeated 50,000 IU regimens taken without checking 25(OH)D can produce hypercalcaemia and kidney damage.
  • Zinc. Above 40 mg a day long term it interferes with copper absorption, eventually causing anaemia and neurological symptoms.
  • Vitamin E. No benefit in the USPSTF review, and separate meta-analyses have linked high doses to increased all-cause mortality and haemorrhagic stroke.

Expensive urine: where the joke holds and where it fails

"Vitamins are just expensive urine" is exactly half right, and the boundary is more useful than the punchline.

True for water-soluble vitamins. Vitamin C and the B vitamins are barely stored. Once plasma concentration exceeds the renal threshold, the surplus leaves in urine. The bright yellow colour after a multivitamin is riboflavin (B2) — a harmless dye and an honest indicator that part of the dose passed straight through. Vitamin C above roughly 400 mg a day is largely excreted, and doses from 2 g upward cause diarrhoea and raise the risk of oxalate stones.

False for fat-soluble vitamins and minerals. A, D, E and K accumulate in liver and adipose tissue. Iron, copper, selenium and zinc are stored too. For this group, excess is not wasted money but a pharmacological load with real consequences — which is why upper limits exist for them and are worth respecting.

Product quality and what the label means

There is a regulatory layer to this too. In the United States, supplements fall under the 1994 DSHEA act: manufacturers do not have to demonstrate efficacy or safety to the FDA before a product goes on sale, and the agency mostly acts after problems appear. Most other jurisdictions treat supplements as food rather than medicine on similar terms.

Independent testing shows what follows. Analyses routinely find gaps between labelled and actual content in both directions — under-dosed active ingredients as often as overages. The worst category is sports and weight-loss formulations, where undeclared pharmaceutical compounds have repeatedly been detected.

What to do in practice

  1. Start with food, not the pharmacy shelf. Vegetables and fruit, whole grains, legumes, fish, nuts and dairy or fortified alternatives cover nearly every nutrient requirement.
  2. Check whether you fall into an indicated group. Pregnancy or planning one, a vegan diet, bariatric surgery, malabsorption, age over 70, long-term metformin or PPIs, an intake below 1500 kcal.
  3. Do not self-prescribe iron or preformed vitamin A. Iron follows a ferritin result; vitamin A in pregnancy follows a clinician and the form on the label.
  4. If you smoke or quit recently, avoid beta-carotene supplements entirely. This is the one item with direct trial evidence of harm.
  5. Add up doses across all sources. The upper limit applies to the total, not to each bottle separately.
  6. Test rather than guess when symptoms appear. Fatigue and hair loss can be anaemia, thyroid disease, B12 or vitamin D deficiency — no multivitamin name tells you which.

Multivitamins are neither poison nor panacea. For a healthy person eating well, they are a modest waste of money carrying a small but non-zero risk in particular circumstances. For someone pregnant, vegan or post-gastric surgery, they are a necessary part of staying well. The useful question is never "do vitamins work" but whether there is a specific reason for you to take a specific nutrient.

FAQ

Do multivitamins help you live longer?+

No. The systematic review commissioned by the USPSTF (O'Connor EA et al., JAMA, 2022) pooled randomized trials covering hundreds of thousands of participants and found no meaningful effect of multivitamins on all-cause mortality, cardiovascular events or cancer incidence in healthy adults.

Who actually needs a supplement?+

Anyone planning or capable of pregnancy needs folic acid, 400–800 micrograms daily. Vegans need B12. People after bariatric surgery need lifelong B12, iron, calcium and vitamin D. Add malabsorption conditions, very restrictive or low-calorie diets, some adults over 70, and people on long-term metformin or acid-suppressing drugs.

Is it true that vitamins just make expensive urine?+

Half true. Water-soluble vitamins — C and the B group — are cleared by the kidneys once plasma levels exceed the renal threshold. Fat-soluble vitamins A, D, E and K and minerals such as iron and selenium accumulate in tissue, so excess there is a toxicity risk rather than a waste of money.

Why should smokers avoid beta-carotene supplements?+

Two large randomized trials, ATBC (1994) and CARET (1996), found higher lung cancer incidence and higher overall mortality in smokers and asbestos-exposed workers taking beta-carotene. CARET was stopped early. The USPSTF explicitly recommends against beta-carotene for prevention.

Did the COSMOS trial show that multivitamins improve memory?+

COSMOS-Web (Yeung LK et al., Am J Clin Nutr, 2023) found a small improvement in an immediate word-recall test among older adults taking a multivitamin versus placebo. It is a secondary sub-study using online testing, and it does not show any effect on dementia incidence.

Can I trust what the label says is in the bottle?+

Not automatically. In the United States supplements are regulated under DSHEA, which does not require pre-market proof of efficacy or safety, and independent testing regularly finds label discrepancies in both directions. Third-party verification marks such as USP Verified or NSF are the practical workaround.
